Dark chocolate is marketed as a guilt-free superfood on the back of cocoa flavanols. The honest headline: the biggest trial of cocoa-flavanol supplements missed its main target, showed only a secondary hint, and — crucially — tested a purified extract, not the sugary bar in the shop.
The flavanols vs. the chocolate
Cocoa contains flavanols, plant compounds with plausible effects on blood vessels. That’s the science behind "dark chocolate is good for you". But a chocolate bar is mostly sugar and fat with a modest, variable flavanol content — processing often destroys much of it — so the health story and the snack are not the same thing.

The big trial, read honestly.
- The main trial result was null — In a large randomised trial (COSMOS) of over 21,000 older adults, a cocoa-flavanol supplement did not significantly reduce the main measure of total cardiovascular events. The headline claim — that cocoa flavanols prevent heart disease — was not confirmed. (hype)
- A secondary hint, not proof — A pre-specified secondary outcome — cardiovascular death — was reduced by about a quarter. That’s interesting and hypothesis-generating, but a secondary result from a null primary trial is not proof. (note)
What this means for your chocolate
- A supplement isn’t a chocolate bar — The trial used a purified flavanol extract. Commercial dark chocolate delivers far less flavanol per gram and comes bundled with sugar and saturated fat — so the trial does not license eating more chocolate. (note)
- Enjoy chocolate as a treat, not a health food — A little good dark chocolate is a perfectly fine pleasure. Just don’t eat it "for your heart" — that framing outruns the evidence. (note)
What we still don’t know
- Is the cardiovascular-death signal real? It was a secondary finding in a null trial; it needs confirmation before it means anything actionable.
- Do flavanols help via blood vessels long-term? Short-term effects on blood-vessel function are described, but durable clinical benefit isn’t established.
