Iron, honestly — essential, common to lack, risky to over-take
Iron is one of the few supplements where both deficiency and excess do real harm. The honest headline: iron deficiency is genuinely common and worth treating — but you should confirm it with a test first, because taking iron you don’t need can be dangerous.
Why iron cuts both ways
Iron carries oxygen in your blood and is essential for energy and clear thinking. Too little causes iron-deficiency anaemia — fatigue, breathlessness, poor concentration. It’s the most common nutritional deficiency in the world, especially in menstruating and pregnant women, infants and frequent blood donors.
But iron is also a nutrient where more is not better. The body has no easy way to get rid of excess, so unnecessary supplements can cause harm — which is why testing comes before treating.
What the evidence supports
Treat a confirmed deficiency; don’t guess.
- Deficiency is common and worth treating — Iron-deficiency anaemia is widespread and reversible; in at-risk groups (heavy periods, pregnancy, infancy, frequent donation) it’s worth checking for and correcting. Fixing it restores energy and function. (established)
- Test (ferritin) before supplementing — A simple blood test (ferritin, plus a blood count) confirms whether you’re actually low. Symptoms like fatigue have many causes — don’t assume iron without checking. (established)
- Don’t take iron you don’t need — High iron intake causes gut upset, and in people with hereditary iron-overload (haemochromatosis) or from repeated transfusions it can damage the liver and heart. This is the rare supplement where "just in case" can hurt. (note)
What we still don’t know
- The best target ferritin for symptoms Some people feel symptoms of low iron before classic anaemia; the ideal ferritin threshold for treating is debated.
- Iron and long-term health at normal levels Whether modestly higher (still-normal) iron stores affect long-term disease risk is an open, contested question.
