Artificial sweeteners, honestly — better than sugar, not a free pass
Non-sugar sweeteners sit in an awkward middle. The honest headline: the WHO advises against using them for weight control, the evidence is low-certainty and confounded, and — importantly — none of that means diet soda is worse than sugar. Water is still the better target.
What the guidance actually says
Non-sugar sweeteners (aspartame, sucralose, stevia and others) provide sweetness with little or no calories. In 2023 the World Health Organization issued a guideline advising against using them for weight control — a headline that got widely misread.
The nuance matters: the recommendation is conditional and based on low-certainty evidence, and it is not a claim that diet drinks are dangerous or worse than sugar.
What the evidence shows
Read the fine print.
- Not a weight-loss tool long-term — The WHO concluded that non-sugar sweeteners don’t deliver lasting reductions in body fat in adults or children. As a strategy for weight loss, they underperform expectations. (emerging)
- The disease "link" is heavily confounded — Observational associations between sweeteners and diabetes or heart disease are muddied by reverse causation — people already at higher risk switch to diet drinks. The WHO itself rated the evidence low-certainty and the advice conditional. (note)
The practical bottom line
- Diet soda still beats sugary soda — The guideline does not say sweeteners are worse than sugar. If the realistic choice is a sugary drink or a diet one, the diet one is plausibly the lesser harm. For people with diabetes, it can be a useful swap. (note)
- Water and unsweetened drinks are the real target — The best long-term move is reducing reliance on sweet drinks altogether, not switching villains. Treat sweeteners as a transition tool, not a destination. (note)
What we still don’t know
- Do sweeteners affect the microbiome or appetite? Some studies suggest effects on gut bacteria or sweet cravings, but the human evidence is early and mixed.
- Are some sweeteners better than others? Whether specific sweeteners (e.g. stevia vs aspartame vs sucralose) differ meaningfully in long-term effects isn’t settled.
