Traditional Chinese Medicine, honestly — sorting the parts that work
Traditional Chinese Medicine is neither ancient wisdom that modern science ignores, nor pure pseudoscience. The honest headline: acupuncture has modest, real evidence for a few conditions; the underlying "qi and meridians" theory has no biomedical basis; and some TCM herbs carry serious, documented safety risks.
Judge the practices, not the philosophy
TCM is a broad system — acupuncture, herbs, cupping, moxibustion — wrapped in a traditional theory of "qi" (life energy) flowing through "meridians". That theory doesn’t map onto anatomy or physiology, and treating it as literally true isn’t supported. But some of the practices have been tested on their own terms, and a few hold up.
The honest approach is to separate the specific practice (does acupuncture help this problem?) from the philosophy (does qi exist?).
Where the evidence is real
Modest, specific, partly non-specific.
- Acupuncture helps some chronic pain — A large individual-patient meta-analysis (nearly 21,000 people) found acupuncture better than both sham and no treatment for chronic pain — back and neck pain, osteoarthritis, headache. The effect is modest and persists over time, but a big part of it is context and expectation, not needle placement. (emerging)
- Acupressure/acupuncture for nausea — Stimulating the "P6" point on the wrist reduces post-operative nausea and vomiting comparably to some anti-sickness drugs, and helps chemotherapy-related nausea. One of TCM’s better-evidenced, low-risk uses. (emerging)
The honest limits
- The qi/meridian theory has no biomedical basis — There is no measurable "qi" or meridian system. Acupuncture’s modest effects don’t require the traditional theory to be true, and "sham" acupuncture (fake points or non-penetrating needles) often works nearly as well — a strong sign the ritual and context do much of the work. - Some TCM herbs are genuinely dangerous — This is the serious part. Some traditional herbs contain aristolochic acid, a proven cause of kidney failure and urinary-tract cancer (classed as a Group 1 carcinogen). TCM products have also been found adulterated with heavy metals or undeclared drugs. "Natural" is not "safe" here — sourcing and regulation matter enormously. (note)
What we still don’t know
- How much of acupuncture is the needle vs the ritual? Sham often performs close to real, suggesting much of the benefit is non-specific (attention, expectation, relaxation) — but the effect on chronic pain is still real and useful to patients.
- Which herbs are safe and effective? Most TCM herbal formulas lack rigorous trials, and quality/contamination varies widely, so blanket claims can’t be trusted.
