CoQ10, honestly — where it helps and where it doesn’t
CoQ10 is a rare supplement with a genuine trial signal — in one specific setting. The honest headline: it shows real benefit in heart failure, but for the popular use of easing statin muscle aches, the evidence is largely null.
One molecule, two very different claims
CoQ10 (ubiquinone) is a compound your cells use to make energy, concentrated in the heart. It’s sold for two main reasons: to support failing hearts, and to relieve the muscle aches some people get on statins. The evidence for those two uses is very different.
What the evidence shows
Strong in one place, weak in the other.
- Heart failure: a real trial signal — In the Q-SYMBIO randomised trial, CoQ10 added to standard therapy reduced major cardiovascular events and cardiovascular mortality in chronic heart failure. A genuine, if not yet definitive, benefit in that specific group. (emerging)
- Statin muscle aches: largely null — For the common use of easing statin-associated muscle symptoms, a meta-analysis found CoQ10 did not significantly help. Some studies show a small effect, but the net picture is at best modest, likely null — despite heavy marketing for this purpose. (hype)
What we still don’t know
- How large is the heart-failure benefit? Q-SYMBIO was encouraging but relatively small; larger confirmation would strengthen the case.
- Is there any subgroup it helps with statins? Overall the statin-muscle evidence is null, but whether a specific subgroup benefits isn’t resolved.
