Metformin, honestly — diabetes drug, longevity hope
Metformin is one of the safest, cheapest, most-prescribed drugs in the world for type-2 diabetes — and one of the most hyped for longevity. The honest headline: it clearly helps people with diabetes; using it to slow ageing in healthy people is a genuine hypothesis, not a proven therapy.
From diabetes drug to longevity candidate
Metformin lowers blood sugar mainly by reducing how much glucose the liver makes and improving insulin sensitivity. It has decades of use, a strong safety record and a very low cost. That track record — plus signals in animals and in observational data — is why it became a leading candidate for an "anti-ageing" drug.
The excitement rests largely on one striking observation and a mechanism story. The observation needs a randomised trial to become fact — and that trial has not happened yet.
What’s actually established
For its real, approved use.
- Effective, safe first-line drug for type-2 diabetes — Decades of evidence: it lowers blood sugar, is weight-neutral, doesn’t cause low-sugar crashes on its own, and is extremely cheap. This part is not in doubt. (established)
- One study: diabetics on metformin outlived matched non-diabetics — A large observational analysis found people with type-2 diabetes on metformin had survival slightly exceeding matched people without diabetes — remarkable, and a big spark for the longevity hypothesis. But it is observational and can’t prove metformin adds years to healthy people. (emerging)
The longevity claim — handle with care
- Anti-ageing in healthy people is unproven — There is no completed randomised trial showing metformin extends healthy lifespan in people without diabetes. It’s a serious hypothesis backed by mechanism and observational data — not an established therapy. Taking it "for longevity" is an experiment on yourself. (hype)
- The TAME trial is proposed, not done — TAME (Targeting Aging with Metformin), championed by Nir Barzilai, is designed to test whether metformin delays age-related disease. As of now it remains under-funded and has not delivered results — so it cannot yet support any longevity claim. (note)
- It may blunt exercise gains — Some evidence suggests metformin can reduce the fitness and muscle adaptations you get from exercise — a real potential trade-off for healthy, active people considering it off-label. (note)
Trade-offs & cautions
- GI side effects and B12 depletion — Nausea, diarrhoea and cramping are common early on (extended-release helps). Long-term use can lower vitamin B12 — worth monitoring. (note)
What we still don’t know
- Does metformin slow ageing in non-diabetics? The central question — and it is genuinely open. Only a large randomised trial like TAME can answer it; until then, longevity use is speculative.
- Does it help or hurt fit people? The possible blunting of exercise adaptation means the risk-benefit for a healthy, training individual is unclear and may even be net-negative.
