NAD⁺, NMN & NR, honestly — the anti-ageing supplement story
NAD⁺ boosters (NMN and NR) are among the most hyped anti-ageing supplements. The honest headline: they genuinely raise NAD⁺ levels in your blood, but the leap from "raises a molecule" to "slows ageing in humans" is not supported by outcomes yet — and the first hard-endpoint trials have been disappointing.
The theory, and the leap
NAD⁺ is a molecule every cell uses for energy and repair, and its levels fall with age. The idea: supplement its precursors — NMN (nicotinamide mononucleotide) or NR (nicotinamide riboside) — to top NAD⁺ back up and slow ageing. It’s a clean, appealing story with real biology underneath.
The problem is the leap from "restores NAD⁺" to "makes people healthier or live longer". Those are very different claims, and only the first has good human evidence.
What human trials actually show
Raises the molecule; benefit is another matter.
- They do raise NAD⁺ and appear safe short-term — In older adults, six weeks of NR raised blood NAD⁺ by around 60% and was well tolerated; single doses of NMN are safely absorbed. The biochemistry works as advertised. (emerging)
- No anti-ageing payoff yet — The healthy-ageing improvements people hope for — blood pressure, arterial stiffness — have appeared only as small, non-significant trends. The one hard-endpoint trial (NR in peripheral artery disease) did find a small, adherence-dependent improvement in walking distance — real, but modest and disease-specific, not evidence of slowing ageing. The honest bottom line
- Promising mechanism, unproven payoff — These are among the more scientifically serious longevity supplements, and research continues. But today, paying a premium for NMN or NR buys a higher NAD⁺ level of unproven consequence — not a demonstrated longer or healthier life. Regulatory status has also been unstable. (note)
What we still don’t know
- Does raising NAD⁺ actually slow human ageing? This is the whole question — and it’s unanswered. We can raise the molecule; we can’t yet show it changes meaningful outcomes.
- Long-term safety and the right dose Short-term use looks safe, but multi-year safety and any optimal dose are unestablished.
