VO₂max & cardio, honestly — the closest thing to a longevity drug
If there were a pill that lowered your risk of dying as much as being fit does, it would be the most valuable drug ever made. Cardiorespiratory fitness — your VO₂max — is the single strongest modifiable predictor of longevity we have. The honest headline: the signal is enormous and real; the "Zone 2" branding around it is partly practitioner-lore.
Why fitness is the strongest signal
VO₂max is the maximum amount of oxygen your body can use during hard exercise — a whole-body measure of how well your heart, lungs, blood and muscles work together. It is not just "how fit you feel"; it can be measured, and it tracks with lifespan more tightly than almost anything else we can change.
In a study of over 120,000 people who did a treadmill test, higher fitness was linked to lower death rates with no upper limit — the fittest had a fraction of the mortality of the least fit, a gap on the scale of major diseases. Low fitness carried a risk comparable to, or greater than, smoking or diabetes. That is why fitness, not weight, is arguably the number to defend as you age.

What the evidence strongly supports
This is about as robust as observational health data gets.
- Higher fitness → lower mortality, with no ceiling — In 122,007 people, each step up in measured fitness meant lower mortality — and being in the "elite" band was associated with roughly 80% lower risk than the low-fitness band. Being unfit carried risk on par with major chronic disease. Association, not proof of cause — but the size and consistency are striking. (established)
- More activity keeps paying off — up to a point — Pooling 661,000 adults, mortality fell ~20% below the guideline minimum of activity, ~37% at 2–3× it, and plateaued near ~39% at 3–5× — with no harm at high volumes. You don’t need to be an athlete; the biggest jump is from "nothing" to "something". (established)
- The dose most people miss: enough, and some of it hard — The standard target is ~150 min/week of moderate or ~75 min of vigorous activity, plus strength work. VO₂max specifically responds to occasionally getting genuinely out of breath — short, hard intervals — on top of easy base mileage. (established)
On "Zone 2" — real physiology, oversold packaging
A popular idea with a solid core and a hyped shell.
- Easy, conversational cardio is genuinely useful — "Zone 2" means training at an easy, still-able-to-talk intensity. It builds an aerobic base, improves fat-burning and mitochondrial function, and is sustainable and low-injury. The underlying physiology is well supported. (emerging)
- The "Zone 2 = longevity" leap is not proven — No trial or cohort shows that Zone-2-specific training lowers mortality more than other exercise. The metabolic benefits are real; the specific longevity claim built on top of them is extrapolation and practitioner-lore, not evidence. Do easy cardio because it works and lasts — not because a heart-rate zone is magic. (hype)
Don’t forget the other direction: sitting
- Long sitting is risky — but activity offsets much of it — In over a million adults, lots of sitting tracked with higher mortality — but ~60–75 minutes a day of moderate activity largely erased the excess risk. So the fix for a desk job is not just "stand more", it’s "move enough". Breaking up sitting still helps. (emerging)
What we still don’t know
- Is the fitness–longevity link fully causal? Fitter people differ in many ways, and you can’t randomise people to decades of high fitness. Genetics set part of your VO₂max. But training clearly raises fitness, and raising fitness is what tracks with lower risk.
- Is any single training "zone" optimal for lifespan? We know a mix of easy volume and some hard efforts builds fitness. Whether a specific intensity distribution extends life more than another is unproven.
